Thank you for your interest in volunteering at Mustard Seed Co-op! Parent volunteers who assist in the classroom 4 times a month (and more if a sub is needed) pay $190/month for tuition.

Background Check Required: All volunteers must complete a background check before volunteering. After you submit this application, we will run the background check on your behalf.

Sibling Policy: Younger siblings who are 4 years of age may accompany parents while they volunteer. For safety and supervision purposes, children under 4 years of age are not permitted during volunteer hours.

By signing below, I authorize Mustard Seed Co-op to obtain a consumer report and/or investigative consumer report on me for volunteer purposes. I understand this may include, but is not limited to, a criminal history check, sex offender registry search, and verification of the information I have provided.

I release Mustard Seed Co-op and its agents from any and all liability arising from the investigation and disclosure of my background information. I understand that a conviction record will not automatically disqualify me from volunteering, but will be considered in relation to the volunteer position.
